Accessing someone else’s information (Proxy Access)

In some circumstances, a parent, family member, or carer may be able to access GP services on behalf of another patient. This is known as proxy access.

Proxy access is not automatic and will only be granted where it is appropriate, lawful, and in the patient’s best interests.

Key points about proxy access

  • Proxy access usually requires the explicit consent of the patient.

  • Where a patient lacks capacity, decisions will be made in line with legal and safeguarding guidance.

  • For children and young people, access will be considered carefully, taking into account age, understanding, and confidentiality.

  • Proxy access can be limited (for example, appointments or prescriptions only) and does not automatically include full access to the medical record.

  • Proxy access can be reviewed or withdrawn at any time.

How to request proxy access

To request proxy access, please:

  • Collect a Proxy Access Application Form from reception (available 9am–5pm), or

  • Contact the Practice for further advice.

All requests are subject to identity checks and review by the Practice before access is granted.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
